Whether to have direct election for every post or whether to have election only for the central provincial and district heads and let them appoint the other office bearers falling under them, is the debate of the day within the PTI.
The first proposal may look more democratic but it falls short on practical functionality. e.g if different thinking people are elected for offices in the district who do not see eye to eye with each other, the party will suffer and every one will put the blame on the other.
On the other hand if the president is elected and he selects his subordinates, it will make a much smoother organisation and the responsibility of the performance of the party will rest on the president who will naturally do his best. So, the lesser intra-party democracy, the better it is for the party.
