Just like, a teacher is called spiritual father to his/her students.
Nazim of any city is the spiritual father of all the city people.

So if someone do hue and cry with some silly complains, as a spiritual father he has right to scold.

The lady in the video, should have thought, how much she is being blessed by the God, that, she is getting the service in 5Rs, which Private Hospitals offer in Rs.20,000.

Also, it should be considered as well. It is so easy to reach NAZIM KARACHI. I wish, all the NAZIM Of Pakistan had as easily reachable as NAZIM Karachi.
