PIA aircraft crashed due to human error: report

ISLAMABAD: Last month’s tragic crash of a Pakistan International Airlines (PIA) passenger plane in Karachi took place due to human errors – the negligence of both the cockpit crew as well as the air control tower – and not because of any technical fault.

There was apparently no technical fault in the aircraft, said the preliminary investigation report submitted to the Aviation Division in a high level meeting on Monday.

PIA flight PK8303, carrying 99 people including 8 crew members, crashed into a densely populated residential area near Karachi airport on May 22 as it was making its second attempt to land. 97 people perished in the accident that also caused considerable damage to the residential areas.
A day after the incident, the Civil Aviation Authority (CAA) had notified an investigation team headed by its Aircraft Accident and Investigation Board (AAIB) President Air Commodore Usman Ghani

The team that also comprised Wing Commander Malik Imran, Technical Investigation Group Additional Director Captain Touqeer, Pakistan Air Force Safety Board Ops Investigator Kamran and Air Traffic Control Ops Joint Director Nasir Majeed was to submit its preliminary report on June 22.

On Monday Air Commodore Ghani gave a detailed briefing to the Aviation Division officials.

According to the report, the CAA officials, the cockpit crew, the control tower and the air traffic control repeatedly made mistakes. Sources privy to the document said the aircraft’s black box has so far not indicated the possibility of any technical fault.

The report said both the speed and the altitude of the aircraft was more than the recommended parameters when the pilot tried first landing. In the first landing, the aircraft touched the ground at the middle of 9,000 meter long runway.

The control tower permitted landing despite greater speed and altitude. The air traffic control also did not provide the control tower with the radio frequency.

The pilot also did not inform the control tower about jamming of the landing gears. It was also wrong decision on part of the pilot to attempt a second landing.

The plane stayed in the air for 17 minutes after the first landing attempt, a crucial time during which both the engines of the aircraft failed.

It said fragments of the PIA aircraft’s engine stayed on the runway for 12 hours but the air site unit did not collect them and later other aircrafts were allowed to land on the runway. This was a violation of the standard operating procedure as it could case damaged to other aircrafts.

According to the report the air traffic control officials should have been relieved after the incident but they continued to perform their duties till 7pm. It said the aircraft’s first engine was installed on February 25, 2019 while its second engine was installed on May 27, 2019.

All three landing gears of the aircraft were installed on October 18, 2014. The fateful plane was 16-year-old and was manufactured in 2004. The plane was included in the PIA fleet in October 2014.

Minister for Civil Aviation Ghulam Sarwar Khan Monday told the National Assembly that the interim report on the plane crash will be presented in the National Assembly on Wednesday. He confirmed that he has received the report and also shared it with Prime Minister Imran Khan.

“The report on the plane accident will be put before the house along with reports on other air crash incidents that occurred since 2010 including Air Blue and Bhoja airlines’ plane crashes in Islamabad, PIA plane crash in Gilgit and other plane accidents,” he said.

Meanwhile, the Aviation Division disowned the report aired by certain television channels regarding provisional inquiry report into the plane crash.

“Media friends are informed that couple of TV channels have been referring to a report regarding crash of PIA flight PK-8303. No such report has been issued by the Aviation Division,” spokesperson Abdul Sattar Khokhar said in a brief statement.

The report, being circulated through the two TV channels, was not from any authentic source, he added.

When I drive a car and I have even one more passenger in the car for some reason I feel alot more responsibility on my shoulders and I become more protective and drive more carefully and ask them to wear seat belts.

Cant imagine how careful I have to be if I have 100+ lives on my shoulders. Its not about religion or morality, its common sense. I'm speechless at the carelessness of control tower and the pilots. This should have never happened. Making a mistake is one thing but this is absolutely criminal negligence by both the control tower and pilot.

The control tower employees and management should be sacked and held accountable. If this control tower had any training they would have never allowed this plane permission to land. The non-seriousness and unprofessional attitude is shocking.

Every sensible person know that was human error we know our country systems each and every institution iz collapsed dilebrately no accountability in any dept Elite class all political parties army beaurucracy judiciary people who ruled 70 years they r responsible for destruction of my pakistan

Imran Khan should replace gradually all PIA pilots and Air trafffic controllers with retired PAF pilots after necessary training or wait for this airline to die.

  • All fake degree holders in PIA should be send to Jail.
  • Fire the judges who approved fake degree holders in PIA.
  • Apply emergency services rules to PIA and dissolve PIA union "PALPIA."
  • Terminate all unnecessary staff

Fake degree or not, as a human being I find it hard that someone would conduct themselves in such a manner if they have 100 lives depending on them. Its horrifying. What kind of people are they?

Imran Khan should replace gradually all PIA pilots and Air trafffic controllers with retired PAF pilots after necessary training or wait for this airline to die.

In conclusion there were many other safer actions that the commercial pilot could have taken ahead of time in all scenarios.

The attitude of of our flight crew i.e Captain and 1st officer is exactly the same as our local bus drivers and the kaleendar (bus conductor) Captain thinks he is phannay khan and flies the plane like W11 bus and poor 1st Officer is there to hear only insults and carry out menial tasks as ordered by Phannay Khan.

I actually don't blame the ATC too much because of the toxic work environment that is rampant in Pakistan and Pilots somehow think they are on the top of the food chain. ATC did warn the pilot and I assuming here but ATC denied permission the Capt would have started to insult him from the radio and might even go up to him after landing to give him a good verbal lashin Ke BC tum apnay aap ko samajta kya hai.

This is also one of the key areas that reforms need to happen. Because of this Air Blue crashed in Islamabad because Mr Phannay Khan pilot all he did was just insult and rubbish his 1st office all during the flight, told the ATC fuck it I'm doing my own thing, fucked up big time ended up killing 100s including himself.

Unfortunately its an Asian culture thing about respecting elders and those senior to you. A korean plane crashed and I think even a Japanese plane all because the 1st officer did not want to disrespect the Captain by correcting his mistakes even though it meant getting killed


And while looking it up I also came across this one. Which has uncanny similarities on of another Korean Air crash

13 June 1991 – (Boeing 727) from Jeju to Daegu, the aircraft performed an unexpected gear-up landing at Daegu. The crew failed to read out the landing procedure checklist and therefore didn't select the gear down option. Subsequent investigation revealed that the pilot instructed the co-pilot to pull the fuse case from the warning system because the repeated warnings that the landing gear was not deployed were, "irritating and distracting," him as he attempted to land. With the warning horn disabled, the Korean pilot brought the plane in and slid down the length of the runway on the central structural rib in the belly of the aircraft.

Agreed. Air traffic control deal with many different airliners all over the world, they should be more powerful, and fully trained to question pilot mistakes during landing and takeoffs.

R u sure current government can bring any positive change in Pakistan in the presence of current judiciary? Unfortunately my answer is negative.

Imran should get rid of all corrupt in Judiciary, Journalism, Bureaucracy, Police, waderas and army by any means. He 68 years old, had a full life and now has nothing to loose.

Imran ! get rid of these criminals by hook or by crook. You will save your name in the history of Pakistan.
