KARACHI: Troubled by an acne breakout that has ruined her wedding preparations, Sadia is on her first visit to a dermatologist. Waiting for her turn at a skin hospital in Saddar, she appears distressed. I get a shock every morning when I look up in the mirror. My pimples are getting worse and I am too scared to think about my marriage scheduled for next week, she says with worry writ large on her face.


Her consultation with the doctor revealed that the breakout was the result of a prolonged use of a beauty cream.
I couldnt believe that the cream caused this. It did give good results for a while and improved my complexion, she recalls, adding that she had started using the cream on a friends advice.


Facing similar predicament is 25-year-old Sadaf. Her face is spoiled by patchy rashes and acne. I have no idea what has ruined my face but the doctor is insisting its the skin whitening cream that I have been applying on my face for the past two months, she says.

Conversations with dermatologists at public sector hospitals of the city suggest that these are not isolated cases. A significant number of female patients report at health facilities with complaints of acne breakouts, pigmentation, skin atrophy (thinning of skin) photosensitivity and excessive hair growth after using fairness/whitening creams.


The kind of acne they have and a detailed history of their cases indicate that they have used some kind of topical steroid-based creams, says Dr Humaira Talat, a senior dermatologist at the Civil Hospital Karachi.


According to her, often these products are purchased from the market but there are also patients who report with damaged skin following their visit to a beauty parlour.


Women going for whitening facials have no idea what kind of creams are being applied on their faces at beauty parlours, she says.


Dr Mahwish Noorani at the Sindh Institute of Skin Diseases in Saddar examines five to 12 such cases daily at her outpatient department of over 200 patients.


She believes that steroid-based fairness/whitening/bleaching creams may also contain harmful metals. There needs to be a scientific analysis of all fairness creams currently available in the market and those found containing steroids or any harmful ingredient should be completely banned. As a doctor, I feel sorry for young women ruining their skin in their desire to get fair.


Explaining how steroids affect the skin, she says: The affected area becomes red with prolonged use [from four weeks onward if a potent steroid is being used in concentrated form]. Small bumps and pustules appear that may be scaly and itchy. At times, enlarged blood vessels develop. The skin becomes sensitive to anything applied to it and the steroid-induced rosacea [skin redness, dilated blood vessels] may become severe when the topical steroid is discontinued.
High cost


According to doctors, patients that also include men, though their number is much less, pay a high cost for using topical steroids on their faces. Not only the treatment for their damaged skin is expensive, the scars left by pimples are permanent.
The skin becomes resistant to regular treatment and takes a long time to recover. At times, the skin pigmentation lasts for good, says Dr Noorani.
She added that patients were asked to immediately stop using the cream, avoid sun exposure and use a good quality sunblock along with medicines to treat acne.
The misuse of topical steroids, she said, was common across the country and there was a need for a regulatory mechanism to check sale of harmful creams.


A 2014 study conducted at the department of dermatology of the Nawaz Sharif Social Security Teaching Hospital in Lahore showed that the unwarranted cosmetic use of topical corticosteriods with or without fairness creams was quite common in facial dermatoses resulting in steroidal dermatitis resembling rosacea.
Seventy-six per cent patients out of 200 (who used such creams and were examined under the study) were female whereas 24pc male.
A majority of the patients, according to the research, belonged to the lower income urban group. Over 30pc patients used potent topical corticosteriods while 50pc used them in combination with various fairness creams and 34pc patients used fairness creams exclusively.
Citing other localized studies including the one by the World Health Organisation, the research pointed out that these creams (names mentioned in the study) had very high toxic levels of mercury and no warning was found on their leaflet or label.
Another study revealed the presence of lead, cadmium, copper and other heavy metals, it said.
Expressing similar concerns, Dr Zahra Yaqeen, the head of the pharmaceutical centre, Pakistan Council of Scientific and Industrial Research (PCSIR), Karachi, noted that a number of easily available fairness/whitening creams had been tested at their facility and were found to have corticosteroids (considered as the last resort therapy) and high level of mercury.
Some other ingredients used in skin lightening/fairness creams are mercury, hydroquinone, ascorbic acid and tretinoin. All of them carry side-effects and, while some of them are suggested not be used at all [steroids and mercury] in cosmetics, others are recommended to be used under medical supervision.
Mercury gets absorbed in the body and damages brain, kidneys and reproductive system. The same case is with corticosteroids, though they also make the skin thin. The latter must be used only on doctors recommendation, senior scientific officer at the PCSIR centre Dr Shazia Yasmeen points out.


When contacted, Pakistan Standards and Quality Control Authority (PSQCA) director general Mohammad Khalid Siddiq said that the authority had revised standards for cosmetics upon receiving complaints against skin lightening products.
A request has been sent to the ministry of science and technology to include cosmetics in the list of technical regulation. Once it is approved, the authority will start checking their sale in the market, he says.
According to the revised PSQCA standards, corticosteriods and heavy metals such as lead, arsenic, hydroquinone and mercury shouldnt be the part of cosmetic raw material at all.
Sharing her experience, seasoned beautician Masarrat Misbah, one of the complainants who approached the PSQCA on fairness cream issue and a social worker working for burn victims welfare, said that she came to know about these harmful creams when she saw their side-effects on the faces of some burn victims.
I was shocked to see their faces. Their skin condition had worsened, she said, emphasising the need for public awareness on the issue.


The fact that prolonged use of creams containing toxic levels of mercury also affects reproductive health should raise an alarm among the public, especially parents and girls.
Apart from the treatment cost, the emotional trauma girls go through after using such creams is huge and affects the whole family, she says, suggesting that girls should eat healthy food and have positive thinking that would make their skin glow automatically.
As a society, we must discourage the mindset that fair is beautiful.

کراچی: کیل مہاسوں نے ماہر امراض جلد (ڈرموٹولوجسٹ) کے پاس پہلی دفعہ آنے والی سعدیہ کی شادی کی تیاریوں کو برباد کردیا تھا۔

صدر میں جلد کے امراض کے ہسپتال میں اپنی باری کا انتظار کرنے والی سعدیہ پریشان نظر آرہی تھیں اور چہرے پر اسی پریشانی کے واضح تاثرات کے ساتھ ان کا کہنا تھا کہ مجھے ہر صبح اس وقت جھٹکا لگتا ہے جب میں اپنا چہرا آئنے میں دیکھتی ہوں، میرے کیل مہاسے بدتر ہوتے جارہے ہیں، اور میں آئندہ ہفتے اپنی شادی کا سوچ کر انتہائی خوفزدہ ہوں۔

ڈاکٹر سے ان کی مشاورت کے بعد انکشاف ہوا کہ ان کے چہرے کا یہ حال طویل عرصے تک خوبصورت بنانے والی کریم کے استعمال کا نتیجہ ہے۔
اپنی سہیلی کے مشورے پر کریم استعمال کرنے والی سعدیہ نے کہا کہ مجھے یقین نہیں آ رہا کہ یہ سب کریم کی وجہ سے ہوا ہے، شروعات میں اس نے اچھے نتائج دیئے اور میری رنگت میں بہتری آئی۔
اسی تکلیف دہ صورتحال سے 25 سالہ صدف کو بھی گزرنا پڑا۔ ان کا چہرہ داغ دھبوں اور کیل مہاسوں کی وجہ سے خراب ہوچکا تھا اور ان کا کہنا تھا کہ مجھے بالکل اندازہ نہیں ہے کہ میرے چہرے کا یہ حال کیسے ہوا، لیکن ڈاکٹر کا اصرار ہے کہ یہ رنگ گورا کرنے والی کریم کے استعمال سے ہوا جو میں گزشتہ دو ماہ سے استعمال کر رہی ہوں۔

سرکاری ہسپتالوں کے ماہر امراض جلد کا کہ اس حوالے سے کہنا ہے کہ یہ چند کیسز نہیں ہیں بلکہ رنگ گورا کرنے والی کریم استعمال کرنے والی خواتین کی بڑی تعداد کیل مہاسوں، پِگمینٹیشن، جلد کا دُبلا پن، حساسیت نور اور بالوں کی ضرورت سے زیادہ نشوونما کی شکایات کرتی نظر آتی ہیں۔

سول ہسپتال کراچی کی سینئر ڈرموٹولوجسٹ ڈاکٹر حمیرا طلعت کا کہنا تھا کہ یہ خواتین کیل مہاسوں کی جس قسم کا شکار ہوتی ہیں اس سے اور ان کی کیسز کی تفصیلات سے ظاہر ہوتا ہے کہ اسٹیرائڈ سے تیار ہونے والی کسی کریم کا استعمال کرتی رہی ہیں۔
انہوں نے کہا کہ اکثر ایسی مصنوعات بازاروں سے خریدی جاتی ہیں لیکن ایسے بھی مریض موجود ہیں جن کی جلد بیوٹی پارلر جانے کے بعد خراب ہوئی، کیونکہ رنگ گورا کرنے کے لیے بیوٹی پارلر سے فیشل کرانے والی خواتین کو اس بات کا علم نہیں ہوتا کہ اس کے لیے کونسی کریمیں استعمال کی جارہی ہیں۔
صدر کے سندھ انسٹیٹیوٹ آف اسکن ڈیزیزز کی ڈاکٹر مہوش نورانی کا کہنا تھا کہ وہ روزانہ تقریباً 200 سے زائد مریضوں کا معائنہ کرتی ہیں، جن میں سے 5 سے 12 مریض اس طرح کے مسائل کا شکار ہوتے ہیں۔
ان کا ماننا ہے کہ اسٹیرائڈ پر مبنی کریمیں میں خطرناک اجزا ہوتے ہیں، جبکہ مارکیٹ میں اس وقت موجود تمام رنگ گورا کرنے والی کریموں کا سائنسی تجزیہ ہونا چاہیے اور جن کریموں میں اسٹیرائڈ یا دیگر خطرناک اجزا موجود ہوں ان پر مکمل پابندی لگائی جانی چاہیے۔
انہوں نے بتایا کہ جن کریموں میں اسٹیرائڈ موجود ہو، ان کے چار یا اس سے زائد ہفتے استعمال سے متاثرہ حصہ سرخ ہوجاتا ہے اور کیل مہاسے اور پھوڑے نکل آتے ہیں جن میں خارش بھی ہوسکتی ہے۔


زیادہ قیمت

ڈاکٹروں کے مطابق جلد کے امراض کا شکار افراد، جن میں مرد بھی شامل ہیں تاہم ان کی تعداد خواتین کے مقابلے میں کئی گنا کم ہے، ایسی کریموں کے لیے بھاری قیمت ادا کرتے ہیں جس کے بعد جلد کو پہنچنے والے نقصانات کا علاج ناصرف مہنگا ہے بلکہ مہاسوں کے علاج کے بعد رہنے والے نشانات بھی مستقل ہوتے ہیں۔
ڈاکٹر مہوش نورانی نے کہا کہ اس طرح کی کریموں کے استعمال کے بعد کرائے جانے والے علاج میں جلد مزاحمت کرتی ہے جس کے باعث بہتری کے لیے طویل وقت درکار ہوتا ہے۔
ان کا کہنا تھا کہ کیل مہاسوں کے علاج کے لیے مریضوں کو سب سے پہلے اس طرح کی کریموں کا استعمال فوری طور پر روکنے، خود کو دھوپ سے بچانے اور دوائیوں کے ساتھ اچھی کوالٹی کا سن بلاک استعمال کرنے کا کہا جاتا ہے۔
پاکستان کونسل آف سائنٹیفک اینڈ انڈسٹریل ریسرچ (پی سی ایس آئی آر) کے فارماسیوٹیکل سینٹر کی سربراہ ڈاکٹر زہرا یقین نے کہا کہ آسانی سے دستیاب رنگ گورا کرنے والی کئی کریموں کا ان کے مرکز میں ٹیسٹ کیا گیا تھا اور ان کریموں میں کارٹیکوسٹیرائڈز اور پارہ (مَرکری) کی بلند سطح پائی گئی تھی۔

پی سی ایس آئی آر کی سینئر سائنٹیفک افسر ڈاکٹر شازیہ یاسمین نے نشاندہی کرتے ہوئے کہا کہ رنگ گورا کرنے والی کریموں میں استعمال ہونے دیگر اجزا میں مرکری، ہائیڈرو کیونون، ایسکوربِک ایسڈ اور ٹریٹائنوئن شامل ہوتے ہیں، ان سب اجزا کے استعمال سے نقصانات ہوتے ہیں، جبکہ اسٹیرائڈز اور مرکری کے استعمال کا تو کاسمیٹکس میں استعمال کا بالکل مشورہ نہیں دیا جاتا۔
رابطہ کرنے پر پاکستان اسٹینڈرڈز اینڈ کوالٹی کنٹرول اتھارٹی (پی ایس کیو سی اے) کے ڈائریکٹر جنرل محمد خالد صدیق کا کہنا تھا کہ رنگ گورا کرنے والی مصنوعات کے خلاف شکایات کے بعد اتھارٹی نے کاسمیٹکس کے معیارات پر نظرثانی کی تھی۔
انہوں نے کہا کہ نظرثانی کے بعد وزارت سائنس وٹیکنالوجی کو کاسمیٹکس کو تکنیکی ریگولیشن کی فہرست میں شامل کرنے کی درخواست کی ہے اور جیسے ہی اس کی منظوری ملے گی، اتھارٹی مارکیٹ میں ان کی فروخت کی چیکنگ کا آغاز کردے گی۔
پی ایس کیو سی اے کے نظرثانی معیارات کے مطابق کاسمیٹکس کی تیاری میں استعمال ہونے والے خام مال میں لیڈ، آرسینک، ہائیڈروکیونین اور مرکری جیسے کارٹیکوسٹیرائڈز اور بھاری دھاتوں کا بالکل استعمال نہیں ہونا چاہیے

As far I know that there is no cream or cosmetic that can whiten the skin, It is all mind games and commercial world is heavily based on people's psychology. In a long run, people waste their money and companies make money. In this case, poor girl has not just wasted money but also damaged the skin.

Skin Lightening Treatments

Skin lightening products -- also known as bleaching creams, whiteners, skin brighteners, or fading creams -- work by reducing a pigment called melanin in the skin. Most people who use lighteners do so to treat skin problems such as freckles, age spots, acne scars, or discoloration related to hormones. It is also a technique used to lighten naturally dark skin.
Skin lightening products do come with some risks. As with any new product, be sure to read the label and know the facts before you buy and apply a skin lightener.

What Determines Skin Color?

Skin color is determined by the amount of melanin in the skin. Melanin is a pigment produced by specialized cells called melanocytes. People with dark skin have more melanin.
How much melanin your skin has is mainly a matter of your genetic makeup. Sunlight exposure, hormones, skin damage, and exposure to certain chemicals can also affect melanin production.
Changes in skin color will often resolve themselves. For instance, tans fade when the amount of direct exposure to sunlight is reduced. But over time, certain discolorations, such as "age" spots or "liver" spots, become more or less permanent.
What Is Skin Bleaching?

Skin bleaching is a cosmetic treatment to reduce the prominence of skin discolorations and even out the color of the skin. You can buy bleaching creams over the counter and by prescription.
Some people apply skin lightener to their entire body to change their complexion, but this can be very risky. The active ingredient in some skin lighteners is mercury, so bleaching can lead to mercury poisoning.

Mercury is a toxic agent that can cause serious psychiatric, neurological, and kidney problems. Pregnant women who use a skin lightener with mercury can pass the mercury to their unborn child.
The use of mercury as an ingredient in skin lighteners is banned in the U.S. However, some skin lighteners produced outside the U.S. may still contain mercury.
How Do Skin Lighteners Work?

Skin lighteners contain an active ingredient or a combination of ingredients that reduces the amount of melanin in the skin where it is applied.

The most widely used ingredient in skin lighteners sold in the U.S. is hydroquinone.
The FDA regulates the use of
hydroquinone in the U.S. Over-the-counter skin lighteners can contain up to 2% hydroquinone. Dermatologists can write prescriptions for lighteners that contain 4%-6% hydroquinone.

It's important to check with your doctor before using a product with hydroquinone and to follow the doctor's directions exactly.

Other skin lighteners use drugs such as steroids and retinoic acid, which comes from vitamin A, as active ingredients. And some skin lighteners use natural ingredients such as kojic acid -- a compound that comes from a fungus -- and arbutin, a compound found in various plants.
Risks of Skin Lighteners

One of the most significant risks of using some skin lighteners is the potential exposure to mercury. One study found that nearly 1 out of every 4 skin lighteners made in Asia and sold outside the U.S. contained mercury.
There are other potential risks of skin lighteners. Those risks can include the following:

  • Prolonged use can contribute to premature aging of skin.
  • Long-term use may increase the risk of skin cancer from sun exposure. Always use sunscreen when using a skin lightener and going out in the sun.
  • Steroids in some skin lighteners may increase risk for skin infections, skin thinning, acne, and poor wound healing.
  • Applying steroids to large areas of skin may put you at risk for health problems related to steroid being absorbed by the body.
  • Hydroquinone may cause unwanted and untreatable skin discoloration (ochronosis).
  • Various bleaching agents, including natural ingredients, can cause skin irritation or allergic reaction.

Special Precautions When Using a Skin Lightener


  • Talk to your doctor before using a skin lightener and ask for specific instructions for the product.
  • Make sure there is no mercury in the product. Mercury is sometimes listed under other names, such as calomel, mercuric, mercurous, or mercurio.
  • Make sure an over-the-counter skin lightener with hydroquinone has no more than 2% of that chemical.
  • If a label lists hydroquinone but doesn't say how much it contains, don't assume it's safe to use. Some foreign products contain more hydroquinone than is allowed in the U.S. and some labels may not be accurate.
If you have any questions about a product you are considering, talk with your doctor or pharmacist to be certain it's safe. Your dermatologist may also recommend other treatment options, such as chemical peels, microdermabrasion, and laser treatments.

  • Pre-cancerous (actinic keratosis) and cancerous (basal cell carcinoma, squamous cell carcinoma and melanoma) skin lesions - due to decreases in the skin's immune function
