The first shaheed in Islam (during the prophethood of Hazrat Muhammed
), were the parents of Hazrat Ammar ibn Yasir (RA)
His mother, Sumayyah Bint Khabbab (RA) was martyred first, followed by Hazrat Ammar's father, Hazrat Yasir (RA).
Hazrat Ammar ibn Yasir (RA), was also martyred in the battle of Siffin, meaning that as a family, they were all martyred in the path of Islam.

But here it is not about the first martyr. It is about the first martyr by "mistaken identity" a.k.a known as "friendly fire" in today's term.

yearned for shahadat. They took part in the battle of Uhud. Though Huzaifah
survived, his father was martyred due to mistaken identity. In fact, Yaman
along with Sabith bin Waqash
had been deputed to defend the camps housing the families of the mujahdeen. But as the battle wore on, they left the camps and plunged into the battle fearing that they would miss a golden opportunity to have martyrdom. Sabith
fell a martyr at the hands of a soldier from the opponents. But Yaman
was chased by a group of soldiers from the Muslim army itself even as Huzaifah
was crying from a distance to alarm them. But Yaman
was dead. Huzaifah
reached as father lay breathing his last. Huzaifah
perceived the gravity of the situation and cried aloud: ‘My friends, May Allah forgive you, and He is a forgiver. Ameen!’
When the Prophet came to know of it, he wanted to give blood money on behalf of the companions. But Huzaifah
refused to take any compensation and said his father wanted to be a martyr, and his wish has been fulfilled. This elevated him in the estimation of the Prophetpbuh.
