This is a very serious issue and is NOT limited to surgical instruments only. Commodities like.... Basmati rice, Cotton bales, Pulses, Himalayan salt, Livestock, Raw gems & stones, Ores, Cement, Fertilizers, Raw marble, Mineral fuels & oils, Edible fruits and nuts, Sulphur, Plastering materials, Raw hides, Leather & allied products.... you name it, all these products are bought by Indians directly from the Pakistani middle men which are shipped in bulk to Dubai. In Dubai Indians have huge warehouses where these products are put in small bags, then put Made in India or Product/Produce of India stamps and sell it to the world making billions of dollars every year.

This is just the tip of iceberg.
