[h=1]Gonorrhoea 'could become untreatable'
[/h] By James Gallagher Health editor, BBC News website

Image copyright Thinkstock Gonorrhoea could become an untreatable disease, England's chief medical officer has warned.

Dame Sally Davies has written to all GPs and pharmacies to ensure they are prescribing the correct drugs after the rise of "super-gonorrhoea" in Leeds.
Her warning comes after concerns were raised that some patients were not getting both of the antibiotics needed to clear the infection.

Sexual health doctors said gonorrhoea was "rapidly" developing resistance.

A highly drug-resistant strain of gonorrhoea was detected in the north of England in March.
That strain is able to shrug off the antibiotic azithromycin, which is normally used alongside another drug, ceftriaxone.

In her letter, the chief medical officer said: "Gonorrhoea is at risk of becoming an untreatable disease due to the continuing emergence of antimicrobial resistance."
[h=2]Soaring cases[/h]But while an injection of ceftriaxone and an azithromycin pill are supposed to be used in combination, this may not always be the case for all patients.
Earlier this year, the British Association for Sexual Health and HIV (BASHH) warned that some online pharmacies were offering only oral medication.
Using just one of the two drugs makes it easier for the bacterium to develop resistance.

The letter, which is also signed by chief pharmaceutical officer Dr Keith Ridge, stated: "Gonorrhoea has rapidly acquired resistance to new antibiotics, leaving few alternatives to the current recommendations.
"It is therefore extremely important that suboptimal treatment does not occur."
[h=2]What is gonorrhoea?
[/h]
_85602038_c0040143-neisseria_gonorrhoeae-spl.jpg
Image copyright CAVALLINI JAMES/SCIENCE PHOTO LIBRARY The disease is caused by the bacterium called Neisseria gonorrhoea.
The infection is spread by unprotected vaginal, oral and anal sex.

Symptoms can include a thick green or yellow discharge from sexual organs, pain when urinating and bleeding between periods. Often the person has no symptoms, however, but can still easily spread the disease to others.

Untreated infection can lead to infertility, pelvic inflammatory disease and can be passed on to a child during pregnancy.

Gonorrhoea is the second most common sexually transmitted infection in England and cases are soaring.

The number of infections increased by 19% from 29,419 in 2013 to 34,958 the following year.

Dr Jan Clarke, the president of BASHH, told the BBC News website: "We're really pleased that the chief medical officer has stressed that gonorrhoea needs this approach to treatment due to the rapid development of resistance.

"We need to protect what we've got and we need to encourage pharmacists and general practitioners to follow first-line treatment."

Dr Andrew Lee, from Public Health England, added: "Investigations are ongoing into a number of cases of anti-microbial resistant gonorrhoea.

"Public Health England will continue to monitor, and act on, the spread of antimicrobial resistance and potential gonorrhoea treatment failures, to make sure they are identified and managed promptly."

[h=1]Gonorrhea[/h]

[h=3]Clinical Presentation:[/h]Gonorrhea, caused by the bacterium Neisseria gonorrhoeae, is the second most commonly reported bacterial STD in the United States. The majority of gonococcal urethral infections in men produce symptoms. However, among women, more than 30% of infections do not produce recognizable symptoms.

Untreated infection can result in complications such as pelvic inflammatory disease (PID), infertility, and ectopic pregnancy. Similar to chlamydia, there are little published data regarding prevalence rates of gonococcal infection in refugee populations arriving in the United States. The single publication on reporting rates of STDs in newly arriving refugees found that 0.2% of more than 2,500 refugees were infected ([SUP]2[/SUP]).

Signs and symptoms of gonorrhea may appear from 1-14 days after a person is exposed to an infected person.
In men, signs and symptoms may include

  • Pain or burning sensation when urinating
  • Penile discharge
  • Painful or swollen testicles
  • Rectal discharge, anal itching, soreness, bleeding, or painful bowel movements

In women, signs and symptoms may include:

  • Pain or burning sensation when urinating
  • Vaginal discharge
  • Intermenstrual bleeding
  • Rectal discharge, anal itching, soreness, bleeding, or painful bowel movements
  • Lower abdominal pain and dyspareunia
Other sites of infection include the eyes (gonococcal conjunctivitis) and pharynx. Disseminated gonococcal infection is associated with arthritis, tenosynovitis, skin lesions, fever or a combination of these signs and symptoms. Skin changes range from maculopapular or pustular to hemorrhagic rashes and lesions. Arthritis and tenosynovitis most typically affect the wrists, knees, and ankles.

Gonococcal infection among infants usually results from exposure to infected cervical exudate at birth. Typically an acute illness develops 2–5 days after birth and may present as ophthalmia neonatorum which may be complicated by a perforation of the globe of the eye and result in blindness. Ophthalmic prophylaxis at birth is effective at preventing this complication. Other manifestations in infants are scalp abscesses, rhinitis, vaginitis, urethritis, arthritis, meningitis, and sepsis.
[h=3]Diagnostic Testing:[/h]Specific diagnostic testing for gonorrhea may be performed on endocervical, vaginal, male urethral, or urine specimens. For screening purposes, urine samples tested by nucleic acid amplification tests (NAAT) are highly sensitive and specific. A Gram stain of discharge or a urethral swab showing intracellular gram-negative diplococci supports the diagnosis and may be sufficient to confirm gonorrhea in symptomatic men.
Because nonculture-based tests do not permit antimicrobial susceptibility testing, in cases of persistent gonococcal infection following treatment, both bacterial culture and antimicrobial susceptibility testing should be assessed. Refugees infected with N. gonorrhoeae are frequently coinfected with Chlamydia trachomatis.

[h=2]Gonorrhea

  • Can Gonorrhea Be Cured?
  • How Is Gonorrhea Treated?
  • What Happens if I Don't Treat Gonorrhea?
  • How Does Gonorrhea Affect Pregnancy and Childbirth?
  • How Can I Prevent Gonorrhea Infection?

Also called the "clap" or "drip," gonorrhea is a contagious disease transmitted most often through sexual contact with an infected person. Gonorrhea may also be spread by contact with infected bodily fluids, so that a mother could pass on the infection to her newborn during childbirth. Both men and women can get gonorrhea. The infection is easily spread and occurs most often in people who have many sex partners.

[h=3]What Causes Gonorrhea?[/h] Gonorrhea is caused by Neisseria gonorrhoeae, a bacterium that can grow and multiply easily in mucus membranes of the body. Gonorrhea bacteria can grow in the warm, moist areas of the reproductive tract, including the cervix (opening to the womb), uterus (womb), and fallopian tubes (egg canals) in women, and in the urethra (the tube that carries urine from the bladder to outside the body) in women and men. The bacteria can also grow in the mouth, throat, and anus.

[h=3]How Common Is Gonorrhea?[/h] Gonorrhea is a very common infectious disease in the U.S. Each year, according to the CDC, there are as many as 700,000 new cases, with less than half of them reported to the CDC. There were 334,826 reported cases of gonorrhea in the U.S. in 2012. Sexually active teenagers have one of the highest rates of reported infections.

[h=3]How Do I Know If I Have Gonorrhea?[/h]
Not all people infected with gonorrhea have symptoms, so knowing when to seek treatment can be tricky. When symptoms do occur, they are often within two to 10 days after exposure, but they can take up to 30 days to develop and include the following:

Gonorrhea symptoms in women

  • Greenish yellow or whitish discharge from the vagina
  • Lower abdominal or pelvic pain
  • Burning when urinating
  • Conjunctivitis (red, itchy eyes)
  • Bleeding between periods
  • Spotting after intercourse
  • Swelling of the vulva (vulvitis)
  • Burning in the throat (due to oral sex)
  • Swollen glands in the throat (due to oral sex)
In some women, symptoms are so mild that they go unnoticed.
Many women with gonorrhea discharge think they have a yeast infection and self-treat with over-the-counter yeast infection drug. Because vaginal discharge can be a sign of a number of different problems, it is best to always seek the advice of a doctor to ensure proper diagnosis and treatment.

Gonorrhea symptoms in men

  • Greenish yellow or whitish discharge from the penis
  • Burning when urinating
  • Burning in the throat (due to oral sex)
  • Painful or swollen testicles
  • Swollen glands in the throat (due to oral sex)
In men, symptoms usually appear two to 14 days after infection.
[h=3]How Is Gonorrhea Diagnosed?[/h]
To diagnose gonorrhea, your doctor will use a swab to take a sample of fluid from the urethra in men or from the cervix in women. The specimen will then be sent to a lab to be analyzed. You also may be given a throat or anal culture to see if the infection is in your throat or anus. There are other tests which check a urine sample for the presence of the bacteria. You may need to wait for several days for your tests to come back from the lab.
Gonorrhea and chlamydia, another common sexually transmitted disease, often occur together, so you may be tested and treated for both.
[h=3]Can Gonorrhea Be Cured?[/h] Yes. Gonorrhea can be treated and cured.

[h=3]How Is Gonorrhea Treated?[/h]
To cure a gonorrhea infection, your doctor will give you either an oral or injectable antibiotic. Your partner should also be treated at the same time to prevent reinfection and further spread of the disease.
It is important to take all of your antibiotics even if you feel better. Also, never take someone else's medication to treat your illness. By doing so, you may make the infection more difficult to treat. In addition,

  • Tell anyone you have had sex with recently that you are infected. This is important because gonorrhea may have no symptoms. Women, especially, may not have symptoms and may not seek testing or treatment unless alerted by their sex partners.
  • Don't have sex until you have completed taking all of your medicine.
  • Always use condoms when having sex.
[h=3]What Happens if I Don't Treat Gonorrhea?[/h]
Untreated gonorrhea can cause serious and permanent problems in both women and men.
In women, if left untreated, the infection can cause pelvic inflammatory disease, which may damage the fallopian tubes (the tubes connecting the ovaries to the uterus) or even lead to infertility. And untreated gonorrhea infection could increase the risk of ectopic pregnancy, a condition in which the fertilized egg develops outside the uterus. This is a dangerous condition for both mother and baby.

In men, gonorrhea can cause epididymitis, a painful condition of the testicles that can sometimes lead to infertility if left untreated. Without prompt treatment, gonorrhea can also affect the prostate and can lead to scarring inside the urethra, making urination difficult.
Gonorrhea can spread to the blood or joints. This condition can be life-threatening. Also, people with gonorrhea can more easily contract HIV, the virus that causes AIDS. People with HIV infection and gonorrhea are more likely than people with HIV infection alone to transmit HIV to someone else.
[h=3]How Does Gonorrhea Affect Pregnancy and Childbirth?[/h]
Gonorrhea in a pregnant woman can cause premature delivery or spontaneous abortion. The infected mother may give the infection to her infant as the baby passes through the birth canal during delivery. This can cause blindness, joint infection, or a life-threatening blood infection in the baby. Treatment of gonorrhea as soon as it is detected in pregnant women will lessen the risk of these complications. Pregnant women should consult a doctor for appropriate medications.

[h=3]How Can I Prevent Gonorrhea Infection?[/h]
To reduce your risk of gonorrhea infection:

  • Use condoms correctly every time you have sex.
  • Limit the number of sex partners, and do not go back and forth between partners.
  • Practice sexual abstinence, or limit sexual contact to one uninfected partner.
  • If you think you are infected, avoid sexual contact and see a doctor.
Any genital symptoms such as discharge or burning during urination or an unusual sore or rash should be a signal to stop having sex and to consult a doctor immediately. If you are told you have gonorrhea or any other STD and receive treatment, you should notify all of your recent sex partners so that they can see a doctor and be treated.
